Case Manager - follow-up and outcomes (remote)

Greenridge Corner Property Management Group is looking for a Case Manager. The job centres on follow-up and outcomes for unit turnovers, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for unit turnovers. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with unit turnovers. • Bilingual English and Spanish. Working arrangement • Contract for three months, with the possibility of becoming permanent. • Remote-friendly, with quarterly visits in Milwaukee. • We reply to every applicant.
