Case Manager - follow-up and outcomes (remote)

Harborlight Field Housing provides affordable homes and support services around Indianapolis. We are hiring a Case Manager to take on follow-up and outcomes. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for repair requests. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with repair requests. • Bilingual English and Spanish. Working arrangement • Contract for four months, with the possibility of becoming permanent. • Remote-friendly, with an occasional day on site in Indianapolis. • Applications are reviewed on a rolling basis.
