Case Manager - follow-up and outcomes (remote)

Stonegate Bright Housing is looking for a Case Manager. The job centres on follow-up and outcomes for resident services, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for resident services. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with resident services. • Bilingual English and Spanish. Working arrangement • Contract for four months, with clear deliverables from the first week. • Remote-friendly, with an occasional day on site in Boston. • We reply to every applicant.
