Case Manager - follow-up and outcomes (remote)

Lumen Union Health Group is looking for a Case Manager. The job centres on follow-up and outcomes for referrals, and it would suit someone who likes steady, practical work. Continuing education is supported. What you will do • Help with applications and appointments for referrals. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with referrals. • Bilingual English and Spanish.
