Case Manager - follow-up and outcomes (remote)

We are growing our team at Bright Basin Health Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 85 people, working mostly from home. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for patient scheduling. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with patient scheduling. Working arrangement • Part-time, about 28 hours a week, with flexible days. • Remote-friendly, with a monthly team day in Little Rock. • We will tell you what to expect at each step of the hiring process.
