Case Manager - follow-up and outcomes (Raleigh, NC)

We are growing our team at Ironwood Basin Care Services and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 174 people based in Raleigh. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for family communication. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • On site in Raleigh. • We reply to every applicant.
