Case Manager - follow-up and outcomes (Raleigh, NC)

We are growing our team at Rivermark Cross Health and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Help with applications and appointments for clinic supplies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish.
