Case Manager - follow-up and outcomes (Raleigh, NC)

Bright Linden Health Group provides primary and specialty care in Raleigh, and we need a Case Manager to lead our follow-up and outcomes. We are busy and kind, and we look after each other. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for patient education. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with patient education. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Raleigh. • Applications are reviewed on a rolling basis.
