Case Manager - follow-up and outcomes (Raleigh, NC)

Nova Cobalt Learning Group is an education provider in Raleigh serving students and families. We are hiring a Case Manager to take on follow-up and outcomes. You would join a large organisation of about 858 people based in Raleigh. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for school calendars. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with school calendars. • Bilingual English and Spanish. Working arrangement • Contract for three months, with the possibility of becoming permanent. • On site in Raleigh. • We will tell you what to expect at each step of the hiring process.
