Case Manager - follow-up and outcomes (Raleigh, NC)

Northwind Maple Community Partners is looking for a Case Manager. The job centres on follow-up and outcomes for grant reporting, and it would suit someone who likes steady, practical work. Mission matters here, and so does looking after staff. What you will do • Connect clients with partner agencies. • Help with applications and appointments for grant reporting. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with grant reporting. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, spread across three days. • On site in Raleigh, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
