Case Manager - follow-up and outcomes (Providence, RI)

Fernwood Willow Community Partners runs community programmes across Providence. We are hiring a Case Manager to take on follow-up and outcomes. You would join a small organisation of about 167 people based in Providence. What you will do • Help with applications and appointments for grant reporting. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Full-time, on a regular day shift. • On site in Providence. • We will tell you what to expect at each step of the hiring process.
