Case Manager - follow-up and outcomes (Providence, RI)

Stonegate Timber Early Learning operates neighbourhood childcare around Providence. We are hiring a Case Manager to take on follow-up and outcomes. Low staff turnover is a goal we take seriously. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for family communication. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • On site in Providence. • We reply to every applicant.
