Case Manager - follow-up and outcomes (Portland, OR)

Fernwood Housing is a nonprofit housing provider in Portland, and we need a Case Manager to lead our follow-up and outcomes. Collaboration with local agencies is constant. What you will do • Help with applications and appointments for resident services. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with resident services. • Bilingual English and Spanish.
