Case Manager - follow-up and outcomes (Portland, OR)

Clearpath Pike Learning Group is an education provider in Portland serving students and families, and we need a Case Manager to lead our follow-up and outcomes. You would join a large organisation of about 803 people based in Portland. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for learning materials. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with learning materials. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • On site in Portland. • We reply to every applicant.
