Case Manager - follow-up and outcomes (Portland, OR)

We are growing our team at Silverline Field Community Partners Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. Mission matters here, and so does looking after staff. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for volunteer shifts. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with volunteer shifts. Working arrangement • Full-time, on a regular day shift. • On site in Portland. • We reply to every applicant.
