Case Manager - follow-up and outcomes (Portland, OR)

Summit Ashton Civic Services is looking for a Case Manager. The job centres on follow-up and outcomes for constituent requests, and it would suit someone who likes steady, practical work. You would join a small organisation of about 63 people based in Portland. What you will do • Help with applications and appointments for constituent requests. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with constituent requests. Working arrangement • Full-time, Monday to Friday. • On site in Portland. • Applications are reviewed on a rolling basis.
