Case Manager - follow-up and outcomes (Portland, OR)

Stonegate Granite Property Management manages apartment communities around Portland, and we need a Case Manager to lead our follow-up and outcomes. You would join a large organisation of about 344 people based in Portland. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for rent collection. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with rent collection. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Portland, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
