Case Manager - follow-up and outcomes (Portland, OR)

Summit Pike Learning Group operates schools and learning programmes around Portland, and we need a Case Manager to lead our follow-up and outcomes. We follow the school calendar and we like people who like learning. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for student accounts. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with student accounts. • Bilingual English and Spanish. Working arrangement • Contract for three months, with the possibility of becoming permanent. • On site in Portland. • We will tell you what to expect at each step of the hiring process.
