Case Manager - follow-up and outcomes (Portland, OR)

Harborlight Rook Housing manages affordable housing communities across Portland. We are hiring a Case Manager to take on follow-up and outcomes. Collaboration with local agencies is constant. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for maintenance stores. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with maintenance stores. Working arrangement • Full-time, Monday to Friday. • On site in Portland, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
