Case Manager - follow-up and outcomes (Portland, OR)

Ironwood Drift Care Services provides in-home and community support in Portland. We are hiring a Case Manager to take on follow-up and outcomes. You would join a large organisation of more than 1,000 people based in Portland. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for licensing. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Full-time, on a regular day shift. • On site in Portland. • Applications are reviewed on a rolling basis.
