Case Manager - follow-up and outcomes (Pittsburgh, PA)

Foxglove Rook Early Learning operates neighbourhood childcare around Pittsburgh, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for meal programmes. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with meal programmes. Working arrangement • Part-time, about 28 hours a week, spread across three days. • On site in Pittsburgh, with some flexibility after the first few months. • We reply to every applicant.
