Case Manager - follow-up and outcomes (Pittsburgh, PA)

Summit Kestrel Community Partners is looking for a Case Manager. The job centres on follow-up and outcomes for food distribution, and it would suit someone who likes steady, practical work. We are small enough to know everyone's name. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for food distribution. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with food distribution. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, afternoons. • On site in Pittsburgh, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
