Case Manager - follow-up and outcomes (Pittsburgh, PA)

We are growing our team at Rivermark Meadow Community Partners Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 195 people based in Pittsburgh. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for benefits enrolment. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with benefits enrolment. • Bilingual English and Spanish. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Pittsburgh. • We reply to every applicant.
