Amberfield Harbor Care Services Partners is looking for a Case Manager. The job centres on follow-up and outcomes for client records, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for client records. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with client records. • Bilingual English and Spanish. Working arrangement • Contract for nine months, with clear deliverables from the first week. • On site in Pittsburgh. • Applications are reviewed on a rolling basis.
