Case Manager - follow-up and outcomes (Pittsburgh, PA)

Rivermark Fern Health Group runs clinics and patient services around Pittsburgh, and we need a Case Manager to lead our follow-up and outcomes. We are busy and kind, and we look after each other. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for patient education. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with patient education. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, spread across three days. • On site in Pittsburgh. • We reply to every applicant.
