Case Manager - follow-up and outcomes (Pittsburgh, PA)

Rivermark Hollow Care Services is looking for a Case Manager. The job centres on follow-up and outcomes for care plans, and it would suit someone who likes steady, practical work. You would join a large organisation of more than 1,000 people based in Pittsburgh. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for care plans. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with care plans.
