Case Manager - follow-up and outcomes (Pittsburgh, PA)

Amberfield Cobalt Early Learning is an early-learning provider in Pittsburgh. We are hiring a Case Manager to take on follow-up and outcomes. You would join a large organisation of about 933 people based in Pittsburgh. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for meal programmes. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with meal programmes. • Bilingual English and Spanish. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Pittsburgh, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
