Case Manager - follow-up and outcomes (Phoenix, AZ)

Foxglove Ridge Early Learning runs childcare centres in Phoenix caring for infants, toddlers and preschoolers. We are hiring a Case Manager to take on follow-up and outcomes. We celebrate small wins and steady routines. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for licensing records. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with licensing records. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, with flexible days. • On site in Phoenix. • We will tell you what to expect at each step of the hiring process.
