Case Manager - follow-up and outcomes (Phoenix, AZ)

Highbridge Timber Learning Group runs classrooms and tutoring across the Phoenix area. We are hiring a Case Manager to take on follow-up and outcomes. You would join a small organisation of about 30 people based in Phoenix. What you will do • Help with applications and appointments for enrolment. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with enrolment. Working arrangement • Full-time, with a steady weekday schedule. • On site in Phoenix, with some flexibility after the first few months. • We reply to every applicant.
