Case Manager - follow-up and outcomes (Phoenix, AZ)

Harborlight Ashton Health Group is a healthcare provider in Phoenix with clinics and support teams. We are hiring a Case Manager to take on follow-up and outcomes. Clinicians and administrators work side by side. What you will do • Connect clients with partner agencies. • Help with applications and appointments for referrals. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with referrals. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Phoenix. • Applications are reviewed on a rolling basis.
