Case Manager - follow-up and outcomes (Phoenix, AZ)

Stonegate Rook Housing is a nonprofit housing provider in Phoenix, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for rent assistance. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with rent assistance. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in Phoenix. • Applications are reviewed on a rolling basis.
