Case Manager - follow-up and outcomes (Philadelphia, PA)

Silverline Vista Community Partners Group runs community programmes across Philadelphia, and we need a Case Manager to lead our follow-up and outcomes. We are small enough to know everyone's name. What you will do • Help with applications and appointments for grant reporting. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Full-time, on a regular day shift. • On site in Philadelphia. • We reply to every applicant.
