Case Manager - follow-up and outcomes (Philadelphia, PA)

Highbridge Timber Early Learning is an early-learning provider in Philadelphia, and we need a Case Manager to lead our follow-up and outcomes. Low staff turnover is a goal we take seriously. What you will do • Help with applications and appointments for meal programmes. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with meal programmes. Working arrangement • Full-time, with a steady weekday schedule. • On site in Philadelphia. • We reply to every applicant.
