Case Manager - follow-up and outcomes (Omaha, NE)

Greenridge Umber Health Group runs clinics and patient services around Omaha, and we need a Case Manager to lead our follow-up and outcomes. You would join a small organisation of about 41 people based in Omaha. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for referrals. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with referrals. Working arrangement • Full-time, Monday to Friday. • On site in Omaha. • We reply to every applicant.
