Case Manager - follow-up and outcomes (Omaha, NE)

Lumen Thistle Learning Group is looking for a Case Manager. The job centres on follow-up and outcomes for family communication, and it would suit someone who likes steady, practical work. We follow the school calendar and we like people who like learning. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for family communication. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, with flexible days. • On site in Omaha, with some flexibility after the first few months. • We reply to every applicant.
