Case Manager - follow-up and outcomes (Omaha, NE)

Redstone West Care Services provides in-home and community support in Omaha. We are hiring a Case Manager to take on follow-up and outcomes. You would join a small organisation of about 117 people based in Omaha. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for family communication. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, mornings. • On site in Omaha. • We will tell you what to expect at each step of the hiring process.
