Case Manager - follow-up and outcomes (Oklahoma City, OK)

We are growing our team at Northwind Meadow Housing and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a large organisation of more than 1,000 people based in Oklahoma City. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for maintenance stores. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with maintenance stores. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Oklahoma City, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
