Case Manager - follow-up and outcomes (Oklahoma City, OK)

Clearpath Pike Learning Group is an education provider in Oklahoma City serving students and families. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for school calendars. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with school calendars. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Oklahoma City, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
