Case Manager - follow-up and outcomes (Oakland, CA)

Westbrook Loom Property Management looks after rental homes across the Oakland area, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for vendor invoices. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with vendor invoices. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Oakland, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
