Case Manager - follow-up and outcomes (Oakland, CA)

Nova Ashton Civic Services provides shared administrative services to local governments around Oakland, and we need a Case Manager to lead our follow-up and outcomes. We follow procedures carefully and explain them clearly. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for budget submissions. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with budget submissions. Working arrangement • Contract for twelve months, with clear deliverables from the first week. • On site in Oakland. • We reply to every applicant.
