Case Manager - follow-up and outcomes (Oakland, CA)

Silverline Hollow Health runs clinics and patient services around Oakland, and we need a Case Manager to lead our follow-up and outcomes. You would join a small organisation of about 47 people based in Oakland. What you will do • Help with applications and appointments for electronic records. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Contract for three months, with clear deliverables from the first week. • On site in Oakland, with some flexibility after the first few months. • We reply to every applicant.
