Case Manager - follow-up and outcomes (New Orleans, LA)

We are growing our team at Fernwood Housing and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 43 people based in New Orleans. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for lease compliance. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with lease compliance. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, with flexible days. • On site in New Orleans. • Applications are reviewed on a rolling basis.
