Case Manager - follow-up and outcomes (New Orleans, LA)

Fernwood Ember Housing is a nonprofit housing provider in New Orleans, and we need a Case Manager to lead our follow-up and outcomes. You would join a large organisation of about 697 people based in New Orleans. What you will do • Help with applications and appointments for repair requests. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with repair requests. Working arrangement • Full-time, with a steady weekday schedule. • On site in New Orleans. • We reply to every applicant.
