Case Manager - follow-up and outcomes (Nashville, TN)

Rivermark Summit Property Management Group looks after rental homes across the Nashville area. We are hiring a Case Manager to take on follow-up and outcomes. You would join a small organisation of about 84 people based in Nashville. What you will do • Help with applications and appointments for lease renewals. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with lease renewals. Working arrangement • Part-time, about 28 hours a week, mornings. • On site in Nashville. • Applications are reviewed on a rolling basis.
