Case Manager - follow-up and outcomes (Nashville, TN)

Greenridge Early Learning runs childcare centres in Nashville caring for infants, toddlers and preschoolers. We are hiring a Case Manager to take on follow-up and outcomes. Low staff turnover is a goal we take seriously. What you will do • Help with applications and appointments for family communication. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • On site in Nashville. • We will tell you what to expect at each step of the hiring process.
