Case Manager - follow-up and outcomes (Nashville, TN)

We are growing our team at Cedar Maple Housing Partners and adding a Case Manager whose main responsibility will be follow-up and outcomes. We value patience and follow-through. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for repair requests. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with repair requests. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, afternoons. • On site in Nashville. • Applications are reviewed on a rolling basis.
