Case Manager - follow-up and outcomes (Nashville, TN)

Ironwood Oak Health runs clinics and patient services around Nashville, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for patient education. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with patient education. Working arrangement • Part-time, about 24 hours a week, mornings. • On site in Nashville. • Applications are reviewed on a rolling basis.
