Case Manager - follow-up and outcomes (Nashville, TN)

Harborlight Sable Housing is a nonprofit housing provider in Nashville. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for waiting lists. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with waiting lists. Working arrangement • Full-time, with a steady weekday schedule. • On site in Nashville, with some flexibility after the first few months. • We reply to every applicant.
