Case Manager - follow-up and outcomes (Nashville, TN)

Bright Loom Community Partners is looking for a Case Manager. The job centres on follow-up and outcomes for donor records, and it would suit someone who likes steady, practical work. You would join a small organisation of about 29 people based in Nashville. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for donor records. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with donor records. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, spread across three days. • On site in Nashville. • We reply to every applicant.
