Case Manager - follow-up and outcomes (Minneapolis, MN)

We are growing our team at Ironwood Cross Health Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 48 people based in Minneapolis. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for clinic supplies. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with clinic supplies. Working arrangement • Part-time, about 28 hours a week, afternoons. • On site in Minneapolis. • We reply to every applicant.
